1. Two years diploma course Auxiliary Nurse Midwifery - ANM

ELIGIBILITY CRITERIA FOR ADMISSION INTO ANM COURSE:
  • 1. The minimum age for the admission will be 17 yrs ( age on or before 31st Dec of that year).The upper age limit is 30 yrs.
  • 2. Minimum qualification: 10+2 passed in arts/science with aggregate of 40% marks recognized by AHSEC/CBSE/ICSE any other equivalent board.
  • 3. Candidates should be medically fit.
  • 4. Simultaneous attendance in other course is not permitted during the course of training.
  • 5. Unmarried candidates are preferred.

2. Three years diploma program General Nursing and Midwifery - GNM

ELIGIBILITY CRITERIA FOR ADMISSION INTO ANM COURSE:
  • 1. The minimum age for the admission will be 17 yrs ( age on or before 31st Dec of that year).The upper age limit is 35 yrs.
  • 2. Minimum qualification: 10+2 passed in arts/science with aggregate of 40% marks recognized by AHSEC/CBSE/ICSE any other equivalent board.
  • 3. 5% of total marks is relaxed for SC/ST candidate.
  • 4. Candidates should be medically fit.
  • 5. Simultaneous attendance in other course is not permitted during the course of training.
  • 6. Unmarried candidates are preferred.

MODE OF APPLICATION

The Prospectus and the Application Form can be obtained from the office of the LIFE CARE DIAGNOSIS Centre located near the Gurdon field. The Application Form must be duly filled and submitted along with the following documents:

  • 1. Qualifying examination certificates and mark sheet of Class X, and XII .
  • 2. 5 recent passport size photograph
  • 3. Character certificates from the school / college principal you studied last/ Parish priest
  • 4. Admit card of class 10 & class12
  • 5. Medical fitness certificate in the given format only
  • 6. Caste certificate
  • 7. Residential certificate / PRC
  • 8. Current annual Income certificate
  • 9. Transfer certificate
  • 10. Copy of undertaking duly signed by the candidate .

Eligibility for Admission to Board Examination

  • 1. A candidate shall be eligible for the admission to the state Council/Board examination if the Principal of the school certifies that :
    • a) She has completed not less than eleven (11) months of the course.
    • b) A candidate must have minimum of 80% attendance (irrespective of the kind of absence) in theory and practical in each subject for appearing for examination.
    • However, the total clinical/practical experience prescribed by the syllabus must be completed before the final council/Board examinations & Diploma is issued.

    • c. Completion of the assigned activities and the practical records,
    • d. A candidate must secure 50% marks in internal assessment separately in each theory and practical.
    • e. The record of practical experience is complete.
    • f. The overall performance & conduct during the academic year should be satisfactory.
    • g. The candidate will appear for theory and practical examination conducted by the Assam Nurse' Midwives & Health Visitors' Council, Guwahati at the end of each academic year.

Failure to Pass the Board/Council Exam

  • 1. Shall have one regular examination followed by supplementary examination in a year.
  • 2. If a candidate fails they shall be promoted to next year.
  • 3. A candidate shall take any number of attempts with a condition that maximum period allowed is 6 years. However all previous papers need to be cleared before appearing in the final examination.

Grading of Examination

Examination shall be graded on aggregate marks of the entire three years of the training programme as follows:

  • Distinction : 80% and above
  • First Division : 70% to 79%
  • Second Division : 60% to 69%
  • Pass : 50% to 59%

Upon completion of the course the graduates receive a Diploma and Registration from the Assam Nurses' Midwives' & Health Visitors' Council, Guwahati.

Selection and Admission

  • ▣ Candidates eligible for admission will be called for entrance examination of I hour duration at +2 level including questions on optional subjects, General knowledge, Values and Morals etc.
  • ▣ Based on the performance at the entrance test and their performance at the board exams, limited candidates will be called for personal interview and medical checkup at the school of nursing.
  • ▣ Candidates called for personal interview shall bring with them all the original certificates.
  • ▣ Candidates will have to make their own arrangements for accomodation during the time of interview.
  • ▣ Basic for selection : Pass in the entrance test, personal interview, medical fitness, higher qualifications and previous records.
  • ▣ The candidates sponsored by Rural Educare Trust is expected to sign and MOU between the Institution & the candidate.
  • ▣ All certificates & DIPLOMA will issued to the graduates, only after successful completion of the internship & the required practical.

GNM - 3 YEARS

b. Mess dues 6,000/- (Monthly) - Before 5th of every month. Delay fine 100/- per day
Seat Blocking Amount 20,000/- (Not Refundable)
Seat Booking 60,000/- (Not Refundable, Seat Blocking amount will be deducted if any)
At time of Admission 87,000/-
Semester dues before 30th April every year - 55,000/-
Semester dues before 30th Oct every year - 55,000/-

(Any delay in semester dues of Rs. 500/- will be added every week)
Note : - Examination fees, University registration fees, Books cost, Uniform cost and other activities fees are not included. You have to pay these fees whenever required.

Please Note:

  • 1. Mess fee is liable to be changed every year.
  • 2. The first year fee is to be paid in installments and has to be completed by the end of academic year.
  • 3. Fees for the second and third year are to be paid in the beginning of each Academic year.
  • 4. The amount once paid to the school is Non-REFUNDABLE.

Specific Rules for Student Discipline :

  • a) Students are required to wear prescribed uniform while attending classes, laboratories and in the hospital during clinical training.
  • b) Students are expected to be courteous, use poilte language and behave with decorum with the faculty members, fellow students and staff of the Institute.
  • c) During clinical training, students are required to maintain punctuality, discipline and follow the rules and regulations of the Hospital Authority.
  • d) Students shall avoid using any insulting, inciting, threatening language when talking with fellow students and should abstain themselves from violence.
  • e) Students shall not talk or act in any manner outside the institution in a way that would bring disrepute to the Institute.
  • f)The students shall be regular and punctual in attending classes and all activities connected with the Institute.
  • g) Gathering in groups at road, entrance, exit and pathways is strictly prohibited.
  • h) The students shall observe strict silence in the class irrespective of the presence or absence ofthe faculty member.
  • i) The campus is Tobacco Free zone. Consumption of any tobacco product is strictly banned inside the campus.
